A Secure Choice

The SecureCare portfolio of products was designed for You, secure in Your choice of coverage and Benefit options. The SecureCare plans provide options from comprehensive and catastrophic health insurance coverage to the new and innovative HSA-qualified plans. With this kind of flexibility, You can chose a plan where You aren’t paying for more coverage than You feel You may need.

A Secure Rate

Your premium rate will automatically be locked in for the first 12 months of coverage, however, Your premium rate can be locked in for 24 or 36 months (depending on plan) – saving You money! Say goodbye to costly annual premium increases. Choosing a SecureCare plan and locking in Your rate gives You long term coverage You can count on without worrying about rising premium costs. Premium Rate Guarantee Period does not apply to any rate change due to: change of address; Benefits; or calculations of premiums mandated by law.

Value-Added Features

SecureCare offers many value-added features to its health plans including national PPO network discounts, Rx discount card, wellness benefits, 12/24/36 month rate guarantees, dental discount program, vision exam/discount program and a foreign travel emergency benefit. Also, if You are treated by an Out-of-PPO provider as a result of an Emergency Sickness or Emergency Injury because an In-PPO provider was not reasonably available, Benefits will be considered on an In-PPO basis until You can be safely transferred to the care of a In-PPO provider.

SecureCare Plans I-VI

SecureCare plans I-VI are designed to provide insurance coverage with a choice of deductibles, coinsurance and co-pay options each with either $2 million of $5 million lifetime coverage per Insured.

The PPO Advantage

With all our insurance plans, You have the freedom to visit the doctor You feel most comfortable with- the doctor You trust. Selecting one of our PPO Providers may allow You to pay less out of pocket. Participating Provider Organizations (PPOs) are a network of physicians, clinics and facilities that have agreed to provide medical care and services at negotiated rates.

One of the biggest assets of PPO plans is their cost containment features. Because the Provider of services has agreed to a negotiated rate this can help reduce Your out of pocket expenses. By utilizing the services of a PPO Provider it enables us to better control our costs thereby helping to keep Your premium more affordable now and in the future.

PPO Features

  • PPO Providers are paid on a fee-for-service basis as their services are used.
  • PPO Providers provide services at a negotiated discounted rate.
  • Insureds that use a PPO Provider pay lower deductibles or copayments than if they use the Out-of-PPO Provider.
  • Insureds do not need referrals to see specialists.

You’ll want to visit PPO Providers to receive the highest benefit percentage, since any expenses at Out-of-PPO Providers will be covered at a lower benefit percentage. For a listing of PPO Providers, go www.ushealthgroup.com or ask your agent.

SecureCare HSA Plans

SecureCare HSA Comprehensive and SecureCare Catastrophic, provide traditional coverage but with the added benefits of being an HAS-qualified single deductible health plan. They offer more affordable coverage and the opportunity for financial savings through a tax-favored Health Savings Acconts (HSA) enabling You to use these funds to help pay for qualified medical services. Use Your HSA funds to pay for many expenses not covered by most traditional plans such as:

  • Lasik eye surgery
  • Eyeglasses/contact lenses
  • Dental treatment
  • Chiropractic care
  • Birth control pills
  • Fertility enhancement

SecureCare HSA Plans provide You with up to $2 million lifetime coverage per Insured with option to increase lifetime coverage to $5 million.

What is an HSA?

An HSA is a savings account with a bank or any other approved financial institution where You deposit money and use the funds for qualified medical expenses today and in the future. The federal government allows tax advantages for the money in an HSA account. You can enroll in an HSA with our partner First HSA, Inc., an FDA-insured recognized leader in HSA’s. To be eligible for a health savings account, you must purchase a qualified health insurance plan, such as SecureCare HSA Comprehensive or SecureCare HSA Catastrophic, not be covered under any other major medical insurance plan or Medicare and be under the age of 65.

The standards for this type of health insurance plan are set by the federal government which determines which deductibles and out-of-pocket amounts are allowed. Many HSA plans allow individuals to choose a higher deductible in order to reduce their premiums. You can save on Your health insurance premium by choosing a high deductible plan. Spending Your healthcare dollars wisely means purchasing only what You need. Higher cost sharing in the form of a higher deductible means a lower cost health insurance plan.
